7.8.8 Amendments to Reports
When a report needs to be changed, amended, or re-issued, all modifications must be clearly identified, documented, and meet all applicable requirements.
7.8.8.1 Identifying Amendments in Reports
When an issued report requires changes, amendments, or re-issuance, any change in information shall be:
- Clearly identified.
- Where appropriate, the reason for the change shall be included in the report.
7.8.8.2 Format for Amendments
Amendments to a report after issue shall be made only in the form of:
- A further document.
- A data transfer, including the statement: “Amendment to Report, serial number... [or as otherwise identified]”, or an equivalent wording.
All amendments shall meet the requirements of this document.
7.8.8.3 Issuing a New Report
When it is necessary to issue a completely new report, the new report shall:
- Be uniquely identified.
- Contain a reference to the original report that it replaces.
